I have pedal problems when I first got my car the pedal was bent (shocker not) so I did the something to this car as my last one I had with a bent pedal took it out striated it out and my buddy welded some extra metal on the side to make it stronger well during all this I think we lost a part after putting the pedal back it there was a lot of play so I got new plastic bushings, Rodney Dickman's Adjustable Fiero Clutch Pedal Banjos, and Rodney Dickman's Isuzu Slave Cylinder The dual seal piston Rebuild Parts. With all this it made the car drivable and I dove it all last year like this. Well spring my arm the moves the throw out bearing locked up and the car couldn’t move so I pulled the tranny and put in another transmission. The car is drivable now but its difficult to get it in reverse and 1st I have the adjustable banjo all the way it could go and you have to press the pedal as hard as you could down to the floor. There is still a lot of play with the pedal. My question is there a bushing or something between the plastic bushing and the bolt that holds the pedal in place

I had the same thing. Take out your driver seat and dive under.

In addition to the adjustable banjo, which is pretty much indispensable, you will find your pivot has loosened. You need two wrenches one for the head and one for the nut. Tighten it until there is no play. Then adjust your banjo so any movement of the pedal causes the slave to move too.

This should solve it. Good luck

I don't know if this will help, but I have a BUNCH of pictures on my transmission page for my Fiero. In the pictures, I have a completely disassembled clutch / brake / gas pedal assembly with bushings and everything, so you can kind of compare / contrast with what you remember putting back together. You might see something that maybe yours is missing?
