I've seen it addressed like this:

Mr. Some Name
123 Streetname
City PR 00716
United Stated

and

Mr. Some Name
123 Streetname
City PR 00716
Puerto Rico

The difference being the country. So what's correct?

As far as I know, US Territories are generally treated as states. Thus, the former should be the correct format.
