The main cause is stop and go traffic where you are just lightly applying brakes ,,also having your foot on the brake pedal..
water can also cause glaze ,Then just driving the car around town makes it worse ,cars that are driven mainly on expressways and hiways build up glaze..because of heat build up,,sort of like the nascar cars heating the rotors red but much less severe of course
also americans have demanded,,insisted fought for,, crumey low quality rotors and there demands were met !! these rotors actually leave pieces of metal in the pads,, the heat these rotors generate cause increased glaze..

The slots in performance/race rotors remove glaze,, the holes mostly lighten the rotors,but this also helps to remove glaze..
if the glaze is not removed it can permeate the pad ..
The quickest way to remove glaze is 2 to 3 fast stops on deserted road,,from 50mph,,...I do not know how this works but it is recomended by pad manufacturers
Most people do not think about pad break in(I don,t)you have to avoid hard stops(Unless a real emergency) Take it easy the first 100 mile ,,you are supose to break them in for 300 to 400 miles screw that
