May be an air/oil explosion that threw the engine out and not a catastrophic engine disassembly, tho it is possible a disaster on the bottom end could have resulted in the oil/air explosion.

At one place I worked about 15 years ago, a kid over-revved a Cat in a cabover Freightliner on a downhill run, threw a couple of rods, knocked a hole in the block big enough to pass a grapefruit through and set the truck on fire, most likely because of the oil mist. The fire went out quickly on its own.
One of the dispatchers kept a rod cap on his desk as a paperweight. You could hardly tell what it was, it looked more like a rock.
