There are thousands of unintentional self inflicted gunshot wounds every year in the US...some turn out to be fatal, some not. 14,000 in 2011...most were by white folks. Many were former military that should have known better.

Familiarity breeds problems.
I've gotten bad the past couple months. Been doing a lot of work on designing some custom stocks, and when I am done scanning whatever it is just lays on the couch. (Unloaded). but not put away, no trigger lock, just tossed aside until I get around to putting it away. Nothing but a bad habit born of laziness and being focused on what I am doing 5 minutes in the future instead of right now. Just me and the dogs home but its still a bad habit to get into.
