Are there specific symptoms that indicate the ICM is faulty?
Can an ICM be tested somehow off the car to see if it's ok?

any auto parts store can test David. I didn't know this till recently.

The Franchise-store test is only as good as the high school drop-out that runs it....
The last time I tried this in Autozone they could not remember where it was or how to run the thing. (The ICM failed, but I got no confidence out of the result, there were a dozen different ways to connect it to the testing machine).

Iffy coil, ICM & coil grounds, Tach Filter and more can kill ICM.
