So I have an Isuzu transmission with my 2.8.

The shift cable has a tendency of catching the throttle cam and holding it at WOT.
What are the best ways you have found to avoid this issue?

Under?
Over the intake?

I have a 2.8 with an Isuzu. I zip tied the cable to the trunk hinge. Everything works fine. Also see this post, from someone else, a little ways down, where there is a picture of the cable routing.

Not the best picture to show what you're looking for, but I routed my Isuzu cables under the cold air hose. It's a little tight, but it looks good and it works just fine. The cables end up arching above and to the side of the throttle quadrant.

Instead of to the left, I zip tied mine over to toward the passenger side on the bracket that's attached to the back of the manifold. It's the one that the sparkplug wires pass down through at the back of the throttle body. But my cables come up and over the throttle body neck so it doesn't put a kink in the cables.

Follow-up:

I finally ended up making a hook out of coat hanger wire to keep the cables attached to the throttle cable bracket.

It sounds ghetto, but it's really not. I'll see when I drive to school tomorrow if it does the job.

Here was my last 2.8/Isuzu swap. If you rotate the rubber air intake down slightly, the cables can run over the top. Notice the one cable by the throttle body is wire tied to the vent tube to keep it away from the throttle cable.
